When the Levee Breaks

OFFICIAL EPISODE DESCRIPTION
Dean (Jensen Ackles) and Bobby (Jim Beaver) lock Sam (Jared Padalecki) in Bobby’s panic room so he can detox from the demon blood.
However, as Bobby sees more seals are being broken he tells Dean they should let Sam out to help them fight the impending apocalypse. Dean disagrees and goes to Castiel (Misha Collins) for help.
Sam and Dean have a big blowout confrontation.

RECAP
In Bobby's demon-proof safe room, Sam waits until Dean opens the door window. Sam asks to be released but Dean insists he has to dry out. He point out that Sam lied to him over and over because he was addicted. Sam insists that he's drinking the demon blood so he can take on Lilith, but Dean doesn't buy it. He tells Sam that he and Bobby will kill Lilith without him and closes the window.

Sam starts to go through withdrawal symptoms. The temperature in the room suddenly drops and Alastair appears, wielding a scalpel and happy to have Sam all to himself. He places Sam on a rack and proceeds to cut open his stomach… and Sam is unaware it's all a hallucination. Upstairs, Dean and Bobby drink while listening to Sam's screams and wondering if he'll survive. Bobby receives a call from Rufus and hangs up, but Rufus calls back and tells him something important.

Sam snaps out of his hallucination of torture, but he sees his younger self appear. Young Sam agrees that Sam is losing his mind and wonders how Sam could do this to him. He blames Sam for running off with Dean and letting Jessica dies, and points out Jessica wouldn't have wanted him to turn down the dark path. Young Sam reveals demon eyes and says there's no way to run from what's within.

Bobby reveals that Rufus has discovered a series of strange events and they're all Seals, breaking faster than ever. Bobby wonders where the angels are then suggests that they should use Sam's abilities. Dean disagrees but Bobby wonders if they love Sam too much.

In the panic room, Sam tries to calm himself when his mother appears to him. He waits for her to accuse him of being a poor son, but Mary says he's doing the right thing and Dean doesn't understand. She tells him to use turn his gift against the demons who gave it to him. Sam worries if the power inside is too strong but Mary insists that Dean is too weak to understand. She tells him not to let anything get in his way and then disappears.

Dean goes outside and calls to Castiel until he comes. Castiel can't speak of what happened in Illinois and tells him to talk about what's going on with Sam. The angel admits that Sam could kill Lilith and stop the Apocalypse. However, if Sam drinks enough blood to kill Lilith, he would be transformed into a creature of evil. Castiel says that Dean is the chosen warrior, and asks if he'll accept his role as the one who will stop the Apocalypse. Castiel vaguely assures him that Sam will be okay and Dean has no choice but to agree, swearing obedience. Castiel tells him that now he must wait until called upon at the proper time.

In the panic room, Sam looks at his hands in horror as black veins crawl across them. He goes to the mirror and sees his entire body covered in the veins. He screams for help while Dean and Bobby discuss Dean's new allegiance. Dean admits he has no choice and Bobby reluctantly agrees with him. Sam stops screaming and they go to investigate. They find Sam convulsing and flailing about, and go in to help him. They restrain him but Dean temporarily freezes up before they restrain Sam.

Once Sam has recovered, Dean asks Sam why he did it. Sam insists he did it for revenge but Dean wonders what the point is. He reminds Sam that God picked him… and Sam realizes it's another hallucination.

Upstairs, Bobby asks Dean again if he's sure they're doing the right thing. He points out they're the ones killing Sam by keeping him locked up, and making him go turkey isn't working. Dean refuses to give Sam demon blood, insisting his brother will die human.

The Dean hallucination accuses Sam of feeling different and outcast, and drinking the demon blood for power and because he was always a monster.

Upstairs, Dean insists that he won't let Sam become a monster.

The Dean hallucination pressures Sam, insisting that Sam is nothing to him and he's a monster. "Dean" disappears and Sam has no choice but to lay there, handcuffed to the bunk. Finally, the handcuffs open on their own and the door opens. Sam walks out and goes upstairs, unaware that Castiel is watching from the shadows. Once Sam is gone, Castiel reseals the door. Upstairs, Sam slips by the sleeping Den and Bobby and goes outside.

On the docks, Castiel is by himself when Anna arrives and asks what he was doing. Castiel insists that he's following orders but Anna warns that Sam is drinking human blood. Two angels arrive to take Anna away, disappearing in a burst of light as Castiel stoically looks on.

Bobby catches up to Sam as he tries to steal a car from the junkyard. He holds a shotgun on Sam but Sam approaches him, insisting Bobby won't shoot him. He points the barrel at his chest and tells Bobby to help if he must. When Bobby doesn't, Sam yanks the shotgun away, knocks out Bobby, and drives away.

Once Bobby wakes up, he and Dean go over the panic room and wonder how Sam escaped. They figure he had help and figure that Ruby is responsible, but Dean isn't so sure. Dean leaves to find his brother, despite Bobby's warning that it won't be easy if Sam doesn't want to be found.

Sam rents the honeymoon suite at a hotel and Ruby comes to meet him. She apologizes for being busy trying to track down Lilith, and admits she didn't figure Dean would lock him up. Sam warns that Dean will soon find him based on his habits, so he rented a honeymoon suite to throw him off. He grabs Ruby and throws her on the bed, and then takes her knife, cuts open her arm, and starts drinking her blood.

The next morning, Bobby tells Dean that the police found the car that Sam took. Two other cars are reported stolen, and Dean decides to track the one that Sam normally wouldn't take.

At the hotel, Ruby notes that Sam's appetite has increased, but she says that it's good because he's getting strong enough to kill Lilith. She warns that there are only two or three seals left, and the angels aren't interfering. Ruby reveals that only Lucifer's first demon, the one he corrupted as a human, can open the seal. Lilith is the first demon, and if Sam kills her then Lucifer never escapes. Ruby has managed to track down Lilith's "personal chef."

At a hospital maternity ward, two nurses check on the babies and discuss a nurse that walked off with two babies and claimed to be possessed. One of the nurse looks at the babies and her eyes go black.

Ruby knows where the demon nurse, Lilith's chef, is located. She warns Sam he'll need more blood than she can give. He hopes that he can repair his relationship with Dean once everything's over.

Bobby gets a report on Sam's car and passes it on to Dean. Weather reports reveal that a nearby town, Cold Springs, are lighting up with demon signs. Before he hangs up, Bobby warns Dean that they need to get through to Sam, not get mad at him.

As Sam leaves the honeymoon suite, Den arrives and enters the room. He attacks Ruby with the demon-killing knife and the two struggle. Dean gets the upper hand but Sam returns to stop him. Sam is glad to see him and wants to talk, but Dean insists they kill Ruby first. Sam tells her to leave and she runs out. Dean insists that Sam would do the same thing in his position, but Sam wants him to come along to dispose of Lilith. Dean agrees as long as Ruby stays behind, but Sam refuses. He doesn't believe Dean is strong enough to kill Lilith, and he wants Dean to trust him just this once. Dean refuses and finally admits that Sam is a monster. Sam punches him and Dean goes to the floor. He gets up, looks at Sam for a moment, and then attacks him. The brothers struggle until finally Sam puts his brother down and then starts to strangle him. He stops at the last minute and tells Dean that he never really knew him, and never will, and then turns and leaves. Dean gasps out that if he leaves, he should never come back. Sam goes out and closes the door behind him.
